Health Insurance
We offer a nationwide PPO healthcare plan which includes a national drug card, dental and vision coverage. This national network CCN is an affiliate of the NCN network. Providers are found at www.ccnusa.com and www.ccok.com. Benefits are the same under both networks and are administered by Healthcare Solutions Group (HSG).

Workers Compensation Insurance
Each W-2 employee is covered under this insurance.
Vision
A preferred provider is not required, giving the employee the flexibility to choose their own Doctor. This benefit covers exams, eye glass frames / lenses, and contacts.

Flexible Spending Accounts
Another great pre-tax benefit! We offer both Medical and Dependent care re-imbursement tax savings opportunities. Enrollment is upon hiring or during the annual open enrollment in December.
Life Insurance
If you elect the health insurance coverage you will have an automatic life insurance "rider" attached to that policy. This benefit is $15,000 Life and $15,000 AD&D.
Professional Liability Insurance, (Malpractice)
Our Insurance covers all of our employees in the field (if applicable).
AFLAC
Voluntary benefits are offered from AFLAC. These are "employee" sponsored benefits, which include: Cancer, Short term disability, Voluntary indemnity, Personal Accident, and supplemental life insurance policies.
